Medication Pick Up
If your child has medication in the Health Office, please remember it will need to be picked up by a parent/guardian by the last day of school. Students are not allowed to transport medication to and from school. Any medication not retrieved will be destroyed. Medication consent forms for next year can be found on the Attleboro Public Schools website under Health Services. Forms are also available in the Health Office.
Graduating Seniors
All graduating seniors will have a copy of his/her school immunization record given to them during graduation practice. This will be provided to the student in a sealed envelope addressed to his/her parent/guardian. It is always a good practice to maintain a record of any medical history, including immunizations.

When to contact the school nurse:
* Whenever there is a question or concern regarding your child's health.
* With any new medical diagnosis.
* Whenever any new medication is prescribed.
* Upon treatment for any serious injury, illness or hospitalization.
* With any new castings, slings, sprains or fractures, use of crutches, boot or wheelchair.
* Every September to renew medication orders for the school year and update medical history.
